Home Stretch WA provides young people leaving Out of Home Care in Western Australia with the option to continue to receive support from 17 ½ until the age of 21.

For young people leaving the care system, the transition to adulthood can be overwhelming. Many face challenges such as securing stable housing, accessing education and employment, and developing the skills needed for independence.
Home Stretch WA bridges this gap by offering a consistent, reliable source of support and the stability needed to thrive as young adults. It ensures young people are not left to navigate this critical period alone, helping them achieve a smoother transition into adulthood.
Home Stretch WA offers young people;
Home Stretch WA Service Providers deliver the Home Stretch WA service to eligible young people across Western Australia.
Department of Communities funds Service Providers and manages referrals through its District Offices.
Home Stretch WA Community of Practice (CoP) is a collaborative network including Home Stretch WA Service Providers, the Department of Communities, young people with lived experience and other key stakeholders who meet regularly for working groups and reflective practice forums to share learnings and continuously improve the Home Stretch WA model.
