Home Stretch WA Community of Practice (CoP)

Who is the Community of Practice?

The Department of Communities has commissioned Aboriginal Community Controlled Organisations (ACCOs) and community service organisations to deliver the Home Stretch WA program across the state. Together they form the Home Stretch WA Community of Practice (CoP).

Regional WA

South West and Great Southern

Mission Australia has partnered with the South West Aboriginal Medical Service (SWAMS) to deliver the program to eligible young people in the South West region. SWAMS delivers the program to eligible Aboriginal young people in the region.

Mission Australia has partnered with Albany Youth Support Association to support delivery of the program in the Great Southern region.

Kimberley 

The following ACCOs deliver the program to eligible young people living in the Kimberley:

  • Broome Youth and Families Hub Inc, servicing Broome, Derby and surrounding communities.
  • Marra Worra Worra Aboriginal Corporation, servicing Fitzroy Crossing and surrounding communities.
  • Jungarni-Jutiya Indigenous Corporation, servicing Halls Creek and surrounding communities.
  • Yawoorroong Miriuwung Gajerrong Yirrgeb Noong Dawang Aboriginal Corporation (MG Corporation), servicing Kununurra, Wyndham and surrounding communities.

Pilbara

The following ACCOs deliver the program to eligible young people living in the Pilbara:

  • Robe River Kuruma – servicing Karratha, Roebourne and Wickham
  • Bloodwood Tree Association – servicing Port Hedland, Marble Bar, Newman, Warralong and Yandeyarra.

Mid-West and Gascoyne

Geraldton Streetwork Aboriginal Corporation (ACCO) delivers the program to eligible young people living in the Midwest and Gascoyne regions.

Goldfields

The Goldfields Indigenous Housing Organisation Inc. (ACCO) delivers the program to eligible young people in the Goldfields–Esperance region and to those living in Wiluna.

Wheatbelt

Yorganop delivers the program to eligible young people living in the Wheatbelt.

For young people living outside of provider’s approximate areas of service delivery, the closest Home Stretch WA providers will discuss the referral and determine whether they will share the support of a young person, depending on the young person’s Country of origin, their preferences for a specific service and the ability of the provider to offer face-to-face support, where practical.

Metropolitan Perth

Four Home Stretch services are available across metropolitan Perth, which includes Bindjareb (Peel region):

  • Yorganop (ACCO) – for Aboriginal young people only
  • Karla Kuliny (ACCO) – for Aboriginal young people only
  • Salvation Army – all eligible young people
  • Wanslea – all eligible young people
 

These services providers will be flexible in response to the transient nature of care leavers and will work with a young person wherever they may be in the metropolitan area.
