Home Stretch WA Community of Practice (CoP)

This training guide is to support Home Stretch WA Transition Coaches & Coordinators in their induction to the Model. 

It consists of video recorded sessions and presentation slides of previous training and practice hours facilitated by the CoP backbone team.  It is designed to be followed from the first Practice Area – Smooth Transition through to Staying On and additional topics. However, Coordinators can vary to meet specific needs of their team.

Training can be completed individually, in 1 – 1 sessions between the Coordinator and Transition Coach or as a team where the Coordinator can facilitate using the training slides. It is recommended to allocate minimum 3 hours per week for onboarding training within the first month to ensure Essential Trainings have been covered.

Note: This training schedule is an interim measure until the Online Learning Modules are released by the CoP in 2024. We are currently developing the new Home Stretch WA Learning Management System (LMS)—a modern, streamlined platform designed to make training and onboarding more concise, efficient, and engaging. In the meantime, you can find previous trainings and practice hour recordings below.
